There is no single answer — it depends on how your vial was mixed. "Units" on a U-100 insulin syringe measure volume, while milligrams (mg) measure weight, so the same 0.25 mg can be very different numbers of units depending on how much powder is in your vial and how much liquid you added. To get a real number you need two facts: the milligrams in the vial and the milliliters of liquid (bacteriostatic water) you mixed it with. This trips up a lot of people, so it is worth slowing down. A milligram is a measure of how much medicine (by weight) is in the dose. A "unit" on a U-100 syringe is just a mark on the barrel — it measures the volume of liquid you draw up. On a standard U-100 syringe, 100 units equals 1 milliliter (mL).
So "how many units is 0.25 mg" really means: how much liquid contains 0.25 mg of my medicine? That answer changes entirely based on how concentrated your liquid is. A strong, concentrated mix means fewer units; a weak, dilute mix means more units — for the very same 0.25 mg.

Worked example (numbers chosen only to show the arithmetic — your vial may differ): Suppose a vial holds 2 mg and you mixed it with 1 mL of bacteriostatic water. Concentration = 2 × 1000 ÷ 1 = 2000 mcg/mL. For a 0.25 mg dose, that is 250 mcg. Units = 250 ÷ 2000 × 100 = 12.5 units.
