Insulin Syringe Sizes: 0.3 mL vs 0.5 mL vs 1 mL

Most people using an "insulin syringe" for a subcutaneous injection are holding a U-100 syringe, and the three common barrel sizes — 0.3 mL, 0.5 mL, and 1 mL — confuse a lot of newcomers. What "U-100" means

U-100 describes the scale on the syringe: it is calibrated so that 100 units = 1 mL. That single fact is the key to everything else. It means:

Almost all consumer "insulin syringes" are U-100. (An older U-40 scale exists but is uncommon; mixing up U-40 and U-100 causes serious errors, so always check the barrel.)

The volume you need to draw decides which barrel is practical. A very small draw is easiest to read accurately on a 0.3 mL barrel; a draw bigger than 50 units simply won’t fit on a 0.5 mL syringe.

Units vs volume: the part people miss

On a U-100 syringe, the numbers printed on the barrel are units, and because of the U-100 scale those units map directly to volume. "Draw to the 20 mark" means 20 units, which is 0.20 mL. You do not need to convert — the syringe already speaks in units.

Where it gets tricky is that a dose is usually expressed in mg or mcg, not units. Turning a mg/mcg dose into a unit mark requires knowing the concentration of your mixed vial — which is exactly the reconstitution math the free calculator does. Remember the unit relationship: 1 mg = 1000 mcg.

Are insulin syringes always U-100?

The vast majority sold for consumer use are U-100, meaning 100 units equals 1 mL. An older U-40 scale exists but is uncommon. Always read the barrel, because confusing U-40 and U-100 causes large dosing errors.

How many mL is one unit on a U-100 syringe?

One unit is 0.01 mL on a U-100 syringe, because 100 units equal 1 mL. So 10 units is 0.1 mL and 50 units is 0.5 mL.

Which syringe size should I use?

That is a practical choice best confirmed with your pharmacist, driven by the volume you need to draw: smaller barrels (0.3 mL) have finer, easier-to-read marks for small draws, while a draw over 50 units needs a 1 mL barrel. This is not a dosing recommendation.

Why are the marks on my 1 mL syringe every 2 units?

Larger barrels often print a mark every 2 units to fit the full 100-unit range. Check your barrel’s increment before drawing, because "the third line" might be 6 units rather than 3.

How do I turn a mg dose into units?
